All students must be age 5 before August 1, 2024 to be eligible to attend kindergarten.

Kindergarten Screening will be held on Friday, May 10 (ALL SPOTS FULL) and Monday, May 13 at West Liberty-Salem Elementary.

To set up a screening appointment for your student:

Email Alysson Cotrell, Elementary Secretary, at In your email, please provide the following information:

  • Parent First & Last Name

  • Student First & Last Name

  • Student Date of Birth (Month/Day/Year)

  • Student Street Address, City, State

  • School District of Residence

  • Preferred time of day (morning or early afternoon)

Screening Process (roughly 1 hour to 1 hour 15 minutes) total:

  • Parent(s) will bring the incoming kindergartener and all necessary registration documents.

  • Children will spend 15-25 minutes in an assessment with a teacher.

  • Teacher will meet with parent(s) to discuss results of the assessment.

  • Children will spend around 30 minutes in playtime/observation time with other incoming kindergarteners.

  • Parent(s) will take incoming the kindergartener to a vision screening.

  • While children are occupied, parents can meet with members of transportation staff, nursing staff (if needed), or office staff.


Proof of Identification

Parent or guardian registering student must provide a photo ID (driver's license) for identification purposes.

Birth Certificate

This is the Certificate of Live Birth from a County Health Department in the state your child was born. The document issued by the hospital is NOT acceptable.

Social Security Card (optional)

If card is lost, a statement from the Social Security Administration with the SSN is acceptable. A copy of your income tax form with the child's SSN is also acceptable if SS card is lost.


A copy of the current immunization record for your child is required. If you do not immunize your child, you must provide reason in writing (dated & signed), OR you may complete a waiver form provided by the school nurse.

Custodial Documents (if applicable)

If parents are divorced/separated, please provide a copy of the most recent custodial documents (must have the court's journal entry date stamp) designating a custodial and residential parent.

Proof of Residency (one of the following)

  • utility bill

  • rental agreement

  • purchase agreement

  • builder's contract (home must be completed within 90 days of enrollment at WL-S)

  • current payroll stub showing WL-S address and WL-S income tax withheld

If you are moving into the WL-S school district with family or friends and you cannot provide proof of residency (above), you may complete a Residency Verification Form from our Registrar.  You will need to have your signatures notarized by a notary public. 


Open Enrollment for Kindergarten is handled differently than for grades 1-12.  Kindergarten students should first be signed up for Kindergarten screening. After signing up for screening, Mrs. Alysson Cotrell will email Kindergarten Registration forms to you, including the Open Enrollment Application.  Please complete all forms and return to Mrs. Cotrell prior to the screening day.  If you also have students in grades 1-12, please wait to send their applications in until May 1st.  Contact Mrs. Alysson Cotrell at with any questions pertaining to Kindergarten screening. 

Please also attend Kindergarten Screening in your district of residence since we have a limited number of Open Enrollment spots available each year.
